CENJARS Launch Cards

Can someone post an image or PDF of the Cenjars Launch Card? I want to print and fill out some ahead of time to save me the trying to fill out a card while I'm also trying to keep my ignitor from falling out. I figure 4 cards fit on an 8-1/2 x 11" sheet if I'm guessing right. Thanks!!
